I am writing to inform you, my family and I will be relocating to Charlotte, North Carolina this upcoming summer. Therefore, I will no longer be seeing patients in clinic after May 24, 2024. 

GPC will maintain all of your health records to assist with your transition of care to a new provider. If you need copies of your medical records or wish to have us release them to another doctor, please call our office as you will need to sign the appropriate authorization form. There is no charge for releasing your medical records.
